How to add a date-picker to a form

It is possible to add a date-picker to a form.

Gutensite Control Panel --> Content --> Forms --> Form Fields (on the left) --> Add New

Under Type of Form Input, select "Date Picker" from the list.

Once the Field has been created, it can be added to forms.

NOTE:  It will only allow one day to be selected.
